Being understood changes what people do.

Someone who feels heard makes the appointment, takes the medication, says the thing they were too embarrassed to say. Not because anyone persuaded them — because being seen for who you are, by someone genuinely interested, makes us "unstuck".

Clinical care needs clinicians. Helping someone act on their own priorities takes a different talent: listening closely, seeing a life from the inside of it, and being authentically interested in the person on the other end.

The technology is a telephone, which nearly everyone already has. No app, no portal, no broadband. The same person every time, on the member's own terms — a real relationship, with room for all the messiness that implies. That is the whole mechanism, and everything else here exists to protect it.
